In the event that the Coach, Trainer, athlete, or official suspects possible concussion during an activity:
- Athlete is removed from play and evaluated by a medical professional.
- Removal from play and/or Incident Report Form should be completed by the coach (link both forms here)
- Return to Play Form should then be completed by a medical professional (link RTP form)
- Athlete may return once the RTP form has been completed and turned in to the school nurse.
Qualified provider at event determines injury was not concussion related
